Access a list of confused drug names identified by the Institute for Safe Medical Practices (ISMP) and includes look-alike and sound-alike drug pairs. The pairs will be listed with the drug name listed first followed by the confused drug name and as well as being listed by the confused drug name followed by the drug name for ease in locating pairs.

An alphabetical list of confused drug names, including look-alike and sound-alike name pairs can be found with a single search in Micromedex® Solutions. The list consists of name pairs that have been involved in medication errors. The list also contains the names that appear on the Joint Commission's list of look-alike and sound-alike names.
